This questionnaire was developed by the Survey Research Center at the Institute for Social Science Research, University of California, Los Angeles. These are questions for the San Fernando Valley Child Guidance Clinic concerning events, behaviors, and attitudes of children impacted by the earthquake. The questionnaire is administered to parents or caregiver. The Sylmar earthquake took place on February 9, 1971. This resource was identified by the NIH Disaster Research Response Program (DR2) for researchers looking for pre- and post-disaster data collection instruments.
