This journal article (subscription required) describes an earthquake and tsunami questionnaire by Hirokazu Iemura et al., Kyoto University. Japanese researchers in Banda Aceh and surrounding areas in Indonesia after the great Sumatra earthquake distributed the questionnaire to people affected by the earthquake and tsunami. The purpose of the questionnaire was to collect information about what happened and what was expected by the affected people to be safe against future earthquakes and tsunamis. The questionnaire consisted of questions asking about their experience during and after the earthquake and tsunami. This resource was identified by the NIH Disaster Research Response Program (DR2) for researchers looking for pre- and post-disaster data collection instruments.
