Resource Submission Form

To suggest a resource (survey, study protocol, data dictionary, or other associated material) for inclusion in the Disaster Research Response (DR2) Resources Portal repository, please fill out the form below as completely as possible. Your suggestion will be reviewed for appropriateness for inclusion. Additional information on field/option criteria can be found in the DR2 Criteria Document.

Before proceeding to the submission form, have documents (research protocols, questionnaires, data dictionaries, consent forms, etc.) and relevant information accessible. Entries will be lost if you leave the webpage.

Resource Description

Title of the Resource. Do not begin the title with “A, An, or The.”
Subtitle of the Resource. Do not include subtitles in the main title field.
The online location of the Resource. If the Resource is not currently available online, please indicate below and upload a file of the Resource at the end of this form (.doc, .pdf., .xls, etc.). Related Resources can also be uploaded at the end of this form.
Please provide a brief summary (maximum of 1,000 characters) describing the Resource. In particular, include information not covered by other fields in this form (e.g., major modules, study design, past use, IRB status, etc). Name and URL of the institution where the Resource was created. (Not the URL to the exact location of the Resource itself).
