Privacy Act Statement
Name, Email, Program, Activity Type, Activity Title, and Brief Summary are required. Name, Email, and Program are primarily collected to verify the submitter is an SRP grant recipient. The Activity Type, Activity Title and Brief Summary are the purpose of this information collection. If you do not provide all of the required information, your update would not be included in SRP announcements, newsletters, or other promotional initiatives. Activity Start Month, Activity Start Year, Link(s) to Resources, Collaborations, Datasets (title, accession number, URL/link, creator/author, repository, PubMed ID, and Publication DOI), uploading files (e.g., photographs), and providing other information in the Brief Summary (e.g., affiliations, professional memberships) is optional. If there is any information you wish to share with SRP staff for informational purposes but do not want shared publicly, please select the “Check this box if you do not want SRP to share the information with other grant recipients or stakeholders (e.g., pre-publication data, results presented in an embargoed paper) (Optional)” checkbox.
